What is the Action plan?

Your Action plan is a personalized caregiving roadmap: actionable next steps and local resources built around your specific situation. It's available to all caregivers and care recipients.

Creating your Action plan

  1. Open Tracker and tap Create an action plan (or tap the suggestion in your iana™ chat).
  2. Answer a short series of questions. We'll only ever ask each question once, so anything you answered during sign-up is already filled in.
  3. We'll craft your personalized plan and let you know as soon as it's ready. You can view it right from the Tracker under View action plan.

Through your employer? Completing the questionnaire also connects you with a Caregiver Navigator.

In a GUIDE program? Your Action plan is pre-filled from your referring provider's information, so it's ready from day one.

Using your plan

Work through your plan at your own pace. Each step includes practical guidance and resources matched to your zip code and needs. Ask iana™ to "See suggested tasks from my action plan" any time.
