Our Dementia GUIDE program provides individualized support based on the severity of the dementia diagnosis and specific needs. The frequency of these visits vary, but most follow-up visits occur once per month. You’ll have direct access to chat online with your Care Navigator and schedule calls through an easy-to-use scheduling platform through ianacare. Our goal is to ensure you have the right level of support at the right time.
