Our Dementia GUIDE Care Navigators do NOT replace your doctors. You will continue being your doctor's patient and reach out to them for medical needs (ex: medication changes, treatment plans, etc.). The Dementia GUIDE Care Navigator is your main contact for all the wraparound support services included in the GUIDE program listed above. They come from diverse backgrounds including social work, nursing, and community health work. Each Care Navigator has specialized training in dementia care and navigation.
