Who may qualify for the Dementia GUIDE program?

Medicare Enrollment: Must be enrolled in traditional Medicare (not Medicare Advantage)
Dementia Diagnosis: Have a confirmed dementia diagnosis
Living Situation: Reside in a home or in the community; those in hospice or a PACE organization are not eligible
